It is a chipboard notecard by Memory Box.  I popped it into my Misti and used Cocoa Fresh Ink by Super Sponsor Impress on the Got Pumpkin set by Brutus Monroe.  The tag was stamped with the STAMPlorations, A Girl and Her Coffee and heat embossed in Zig Apricot embossing powder.
